I am doing a form 4 for with a trust for #15 "Transferee's certification". For name of tranferee do I put my name or the trust name and what do I put for signature transferee.

You put the full name of the trust since it is the "transferee" in box 2a.....you then sign your name:  "GMG, Trustee" on the signature line.
