Form 4 asks for transferee address, and I believe they mean physical address.
Form 1 says mailing address, then has another box for a physical address if the mailing address is a PO box. The difference being that a form 4 will go back to the transferor and a form 1 will back to the applicant/maker. In either case, they appear to want the physical address. |
