if I understand correctly, its not a bad idea to have my form 5's already completed and in my files (with my form 1s and form 4s) in case something happens to me.  so that this way, the process is started (if not at least the intent is shown) to get my goodies to my heirs in the event of my death.

all my forms (1 & 4) are individual.

this correct?

now.....I hate to be "this guy" but can anyone point me in the right direction for a "sample"?  or can they email me an XXX'ed out version of their own?
looking thru the form, its a little more complicated than 1s and 4s and I want to make sure its done correctly.

thanks.....I'm familiar with this (have used 3x in the past)

I think it is simpler. (is that a word)..basically if neither of you are a FFL or SOT fill out 2-4 to match your exisitng F4 or F1, skip 5,6,7,8

I leave 9,10,11,12 for the executor and 13-17 for the the rightful heir, either you can fill them out now or leave them for the executor to complete

Oh yeah, 3.e needs filled in later....
