im looking to get a suppressor on my m4 what form do ineed and where do i get one?
and where will this be sent, thanks

First you need to find a C3 dealer in your area.  Here are several mfgrs of suppressors -- if you contact them, they can point you to a local dealer.

The dealer will guide you through the process -- you will be filing a Form 4 to transfer the suppressor to you.  The one time Federal tax is $200.

so if i pay the $200 1time fee, what do i have to pay to get another 1 on a different gun?
and this form and fee i fill out does it also cover a sorter barrel also?

You pay the $200 tax for every title II item -- a short barrel rifle (SBR), a short shotgun, a silencer, a machine gun etc (an AOW is a $5 tax, but that is another matter).  The one suppressor covered by your one $200 tax can be used on different guns as long as the caliber is compatible with the suppressor.  So if you want to SBR an AR-15 and put on a suppressor, it would be two form 4's and 2 x $200 = $400 tax.

but could i get a 11.5" barrel with a 7" can
or a 14.5" barrel with a 5" can

There are a lot of things that you can do, but to be legal, you have to register the lower as an SBR if you are planning on using any barrel with a length less than 16".  If the suppressor is permanently mounted to the barrel and the barrel + suppressor assembly is over 16" you are good to go with just a suppressor Form 4 and a single $200 tax.

You dont want to weld a can to your barrel.  You will regret doing this to save $200 if you do it.  Plus you have to pay to have it welded so the savings is even less and you can use that silencer on other uppers either if its welded.
