I can talk to my regular doctor but don't think he would prescribe, he had me on a bunch of OTC meds(ashwaghanda, tongkat ali, zinc glycinate, DHEA) to try. It didn't seem to make much difference. My previous labs in past few years had test in the 325-365 range. Reading around these numbers seem not great, not terrible?

I called what I thought was one nearby clinic, Renew Vitality, turns out they just telemedicine anyway and just forwarded to a sales person who was like "yeah you're not going to feel good at 300 you need to be 900-1200", i asked about people feeling different at different levels and he acted like that was a dumb question.

I would say those are low, I believe I was in the 450s-550s before starting.
As been mentioned before through the thread, it is not about the numbers but more how you feel.

I bet those supplements are costing 10x more overall a month then TRT.

With regards to "you need to be 900-1200", it goes back to not focusing on a number.

Already solved the problem myself but it was just a shitty move and waste my time, I want my old geezer doc back

Does this shit happen in the US?

I know old post but made me laugh.   Yes I've had issues with doctors and had to shop around I am basically self managing with a NP writing the RXs for me.

Looking at other options and advice. My plan is if i do this to try 3-6 month of lifting, eating better, etc. to go with it, and see if I can make good gains and feel better.

Well no TRT action for me. I was honest on intake form. I had a AFIB issue 15 years ago. They will not treat me unless I get a Dr release. I do not have a PCP as I am healthy. My cardiac doc one per year, check up, no issues see you next year. I understand the risk of TRT and hematicrit. I did TRT in 22/23 with only 100mg/week. I stopped as it did not help much.

Any advice ? I really want to feel better and get my T up. My levels are really low like <140. Try another tele health place and don’t mention AFIB?

Fun fact, the Olympic training center was placed here in Colorado Springs because elevation and blood chemistry have a thing together.    The recommended 'crit level is flatlander bullshit.  I was "too high" at regular blood donations even before T.

Look up this group on Facebook. They have a lot of medical providers who can answer your questions and possibly treat your symptoms.

It really is a quick and painless process, and holy shit the prices are so affordable over typical TRT clinic prices, even dealing with the domestic suppliers as opposed to the Chinese ones

Wanted to give a heads up on an awesome sale on this insulin pen. -sale over, new link-

Insulin pens are a great way to administer peptides...and you can also use them for TRT/oils!

No more fucking with syringes constantly, just fill it up and you're good for quite a while/3ml. It's also much more socially low-key than actual syringes.

This is the pen, it's about half off regular prices. The advantages: it's an automatic pen, meaning you dial your dose then push the button and it continues to push the plunger. Not as critical with peptides especially smaller doses (.1-.2ml), but if you're using them for oils, it's much easier than trying to push a manual pen for oil (very stiff). Another advantage...it can do up to 60iu (.6ml) in a single twist. The Owen Mumford Autopen could only do 20iu and is now discontinued. Where it is NOT good... injections UNDER .1ml/10iu...the spring force doesn't build enough and you'd have to twist the end cap to drive the dose...this is where the Owen Mumford shined.

I just found this on AliExpress:  $36.63 | Chonghe Lin Insulin Injection Pen Chonghe Lin M30 Diabetes Home Automatic Injection Pen https://a.aliexpress.com/_mtNjHOh

For peptides, these are probably the most advanced pen tips on the market, literally painless. No Rx needed, hopefully they ship to your state, they don't to mine. You can always buy an off brand from Amazon if you're in that boat and see what you like:

https://www.adwdiabetes.com/product/25526/bd-nano-ultra-fine-pen-needles-32g-4mm-2nd-generation-box-of-90

For oils/IM injections, you're going to potentially need a larger gauge, and definitely a deeper/longer needle. I don't have much body fat, but I also don't mind oil sub-q. You definitely don't want to do oil shallow sub-q, the prior needles are far too tiny. I've been using the following ones with no issues:

Amazon Product
  • UNIVERSAL DESIGN: Easy twist-on and compatible with all Type-A pen injectors

I have not been able to find 1/2 30 or 31g pen tips that are not Rx, so 8mm 31g was my compromise. Let me know if you run across anything.

And finally the cartridges you fill and use. These are the cheapest I've found. They're also on sale, normally a few bucks more. But 50ct for $14 is pretty killer.

https://a.aliexpress.com/_mtFMIvr

You'll need to use an insulin needle with the plunger pulled or a syringe needle as a "vent needle" so you can fill them. Here's a video showing how all this works:

Would appreciate some feedback from some experienced TRT folks on here.

47
6'0 225lbs
Cardio (swimming and bjj) and lifting multiple times per week. Working on diet.

I've been on TRT for 1 year, starting at 90mg per week (MWF injections) for 3 months, the rest of the time I've been doing 120mg per week. To put it bluntly, my dick has stopped working. A number of months back I started experiencing decreased penile sensation with serious ED issues (with wife AND by myself) happening for the last 1.5 months or so. I've been on a low daily dose of Tadalafil (5mg) for a long time. This is kind of wrecking my world right now

I'm at the point where I don't quite know what direction to go from here. My provider doesn't really provide much guidance and allows me to 'drive' any consultations with him. I explored the idea of trying a low dose AI (Aromasin) to see if it helps with the ED symptoms but am second guessing this A LOT. Should I try lowering my dose to 100mg a week instead of attempting to DIY a AI dosage on my own? I was a 'slow' (don't what the technically correct term is) responder when I first got on TRT (90mg only got my total T to the 400's from 270). Here are the following lab values from my latest lab draw (2 days after last injection):

Hemoglobin                  15.7 g/dL
Hematocrit                    46.1 %
Total T                            724 ng/dL
Free T                             164.3 pg/mL
Estradiol                         39 pg/mL             (E was 22 before I started TRT)
Prolactin                         17.3 ng/mL
SHBG                              21  nmol/L
Progesterone                 .6

I'd greatly appreciate any thoughts on these issues. Without being an expert I'm guessing the Estradiol issues are causing my ED. I have heard that elevated prolactin can cause sensitivity issues. Thought about lowering dose then adding HCG but the cost is making me now want to go down that road.

At this point I'd be willing to pay to have a consultation with an expert to try and fix this.

Thank you in advance for any advice anyone can offer

could it be your estrogen or prolactin? It's possible, but those numbers are still pretty low. You could be having psychological issues or you could be getting older and it's blood flow issues

Best thing to do would be to go to a urologist and find out

You could try hCG for sensitivity, it's pretty cheap at $119 for 10,000 IUs from Optum Rx.  Or even cheaper in the gray market.

i switched esters and it made a difference for me.

That's really bad advice, caber is a strong drug that can have severe side effects, and his prolactin is not that high, it's not even outside the reference range. If he's working with a provider his should bring the issue to their attention and get medical professional advice. If he's DIY, he should try a non-suicidal AI like Arimidex cause it's harder to screw up and less likely to crash his estrogen, and see if dropping his estrogen a touch helps. Like .25mg once or twice a week and go from there. If he needs estrogen management longer term, if he wants he can learn how to use Aromasin and get it titrated right.

Now he specifically mentioned a loss a sensitivity, so HCG is my first thought to address that symptom. But that may have to go hand in hand with estrogen management.

If he's on finasteride or dutasteride, some people find the DHT blocking effect can reduce penile sensitivity, but if he's on it, I'm sure the last thing he'd want is to put DHT cream on his junk lol.

Testosterone alone doesn't make everyone's sexual performance perfect. Sometimes it requires levels of different hormones to be optimized, and sometimes a little polypharmacy.

You certainly lose the production of certain neurosteroids when only on exogenous testosterone which can effect mental and sexual performance. Some people are very sensitive to that, most are not. HCG promotes intratesticular testosterone production, and the downstream production of those neurosteroids.

If nothing common or obvious worked, AI, HCG, more/less test, neurosteroid supplementation, and he wanted to see what lowering prolactin did, then something mild like P5P (B6) would be a first stop.

His test dose is very moderate. Even people blasting tren try caber when they don't need it and fuck themselves up.

A simple thing to try is some DIM.
Take 200mg in the morning and 200 at lunch.
Buy 1 bottle and give it 30 days.

Not on finasteride or dustateride. Already taking P5P. Definitely considering lowering my dose to 100mg per week then adding HCG, then go from there.

My provider is mostly useless so if anyone can recommend a stellar telehealth provider than can dig into this issue and provide some guidance I'd be greatful.

Thank you for the response.

Let's say you've got your routine dialed. Bloodwork is good, all hormones are either in range or you're having no side effects, but you decide for some reason you want to run HCG (to get your testicles back for example). You get on HCG and boom, estradiol is way off, you're finding yourself crying at commercials, your routine is no longer working smoothly like it was. I'm not saying this happens 100% of the time, I'm just saying it's a significant variable that often causes things to shift.

When my TRT doctor offered it to me I declined, I wanted to see how things would go without it. One part of his job is to sell me products, and even he said something like "that's reasonable, HCG is a huge variable to add in right now".

A lifelong friend is the one who told me about the crying when his hormones got wacky on HCG. At first he liked it, all the emotions. Like he'd been missing out on feelings for years. That didn't last long 😂. Maybe you ran an AI before and knew the dosage that worked, or didn't need to take an AI at all. Well, add HCG to the mix and you may have to reassess.

That didn't last long . Maybe you ran an AI before and knew the dosage that worked, or didn't need to take an AI at all. Well, add HCG to the mix and you may have to reassess.

Be aware, HCG adds a huge variable. I'm hesitant to even try it.
How so?

Let's say you've got your routine dialed. Bloodwork is good, all hormones are either in range or you're having no side effects, but you decide for some reason you want to run HCG (to get your testicles back for example). You get on HCG and boom, estradiol is way off, you're finding yourself crying at commercials, your routine is no longer working smoothly like it was. I'm not saying this happens 100% of the time, I'm just saying it's a significant variable that often causes things to shift.

You seem to have an irrational fear as the result of a bunch of anecdata.

It's a single variable.

A variable that already exists from the administration of testosterone, but potentially in greater magnitude.

His shit certainly isn't dialed, his cock isn't working.

It certainly, at today's juncture, is an upsell from clinics because they charge out the anus for it. It's an unsuccessful upsell at 99% of places because it's so goddamn expensive now through 95% of clinics, and then they pivot to bullshit like gonadotropin or clomid because who doesn't love eye floaters or blood clots.

For people where it's covered by insurance, or people who can source, it's dirt cheap.

It provides the missing puzzle pieces that TRT alone can't provide. If you read my above post, the vast majority of guys, especially much older guys, don't need those finer points. Just getting their test up did the heavy lifting. But there are some people, and the previous poster may be one, where it helps.

And this talk of it being a significant variable and hard to manage...I got irritable...I added an AI...I got irritable months later... increased frequency of AI.

Find symptom, address symptom, fix symptom.

Honestly more people should try it (if they can understand how to afford it).

IMO 70%+ of the positive psychological benefit I feel is from HCG, to the point where I probably should have tried HCG monotherapy for as long as possible first.

Has anyone looked into using Function Health?

I'm wondering how this would be for an overall health outlook, it includes a LOT more tests than just TRT related, which might help identify other things too. How does the "male health" test battery look? Would I need any of the add-on/cost extra tests for a meaningful analysis, or would they just be a bonus?

I'm still trying to understand exactly what I need, everyone seems to have different packages.

For $499 per year, a Function membership gives you:

100+ lab tests at the start of each year—no hidden costs, no insurance required.
60+ follow-up tests at 3–6 months to track changes.
Insights from top doctors based on your results.
A written clinician summary to help you understand your data.
Access to re-test at transparent prices when needed.
Option to purchase additional tests, including Lyme, STDs, and allergies.
Access to groundbreaking tests, like the Galleri Multi-Cancer Early Detection test.

They are offering testing, they do not offer action or treatment based on results. Basically just a bunch of data and a whole bunch more money.

If you want advanced more holistic testing, and a medical practitioner to assist actioning on that data, Mareck would be the outfit.

I just did it, very happy with the process so far. I'm sure there are other/cheaper options but really like the depth they give you in each report and how easy it was with my local Quest labs. I have more data on my cholesterol that was helpful too compared to the basic metabolic panel from my primary doctor. I don't think you need any of the add-ons unless you might be looking for possible explanation to an otherwise unsolved issue.

Just don't pay full price, there are coupon codes for at least $100 off and if you can't find one message me I can give you a referral code.

My issue is that in my area it seems to be difficult to find someone who isn't on either side of the spectrum...

PCP who doesn't think TRT is necessary or is dangerous, vs clinic who just wants to sell TRT to anyone who comes in the door.

I am hoping to get lab results first, then try to get a realistic, unbiased review of it and opinion on what course of action I should take.

I would also be willing, once I get things figured out, to source for myself rather than paying an inflated subscription price to a clinic.

I'll check out Marek, though.

If that's what your looking for, that would be my first thought. They built their entire program around comprehensive testing. They are not cheap, and they're certainly in the business to sell stuff and make money, but a holistic view and testing send to be more of their core approach.

If they stick you with a health coach run away as they're not allowed to give you any medical advice or answer medical questions as that's unethical, and most likely illegal.   And that's what I heard they do a lot and that's not coming from me but from other TRT providers.

Just had my first TRT dose yesterday at my Men's health clinic.

They started me off with 140mg/wk... 70mg twice weekly.

I opted for the sub q injections versus the ointment or the pellet.

It's $180/mo and $99 every 4-8 months for bloodwork.

Probably stick with the clinic for a year, and then go UGL and save a ton of money.

ETA... 1.5 years ago I had labs done and test was at 231. PCP said to get fucked you're in range.
Dialed in my diet and joined a gym... Now I down from 285 to 196-200lbs, and my test went to 641.
I got in TRT for QOL/ weight lifting performance and all the other benefits.
My weight loss and muscle gains kinda plateaued and stalled out.
Going for performance optimization

I'd assume.

I don't see how that's much different than most TRT clinics where you're working with a "patient care coordinator" and there's a medical professional in the background checking things over and signing off on stuff, but I don't really follow clinics closely. For someone specifically not looking for drugs, I read that Mareck is the least like a TRT mill and the most looking at overall health

The clinic I've used I've always talk to a medical professional, a MD or NP. Like I said it's unethical possibly illegal for someone who's not a medical professional to be giving you any medical advice, and that's what their health coaches are, not medical professionals.  And why would you pay those prices not to talk to a medical professional?

Frankly, same reason people find most GPs and urologists terrible for TRT needs. There are tons of well educated people skilled in this space beyond letters next to their name (that are supervised). How many bodybuilding top bodybuilding coaches are actual doctors? Very few.

I met an NP the other day that does pellets for men and women like candy. Had no idea what an AI was. Was totally pro TRT but had little understanding.

We are talking medical clinics that are licensed.  Heath coaches are not licensed health care providers.  Do you actually think the dr is standing over their shoulder while they give you results and answer your questions?

Those Drs are not good because they didn't get specialized training in it.

There is a reason when you call your Drs office to get results and ask the person giving you them what it means, they say the dr will call to explain it.

And the only thing a patient care coordinator did was schedule me with the Dr and get my payment info.

Been feeling shirty for about a year, thought it was just work and life stress but my mental has leveled out lately while.my energy still sucks. Plus I've put on 25lbs since Christmas with no significant diet change wtf.

Went to a functional clinic last week, got results yesterday. They put me on guard right away by telling me I have an autoimmune disease called Hashimotos despite being asymptomatic aside from low energy. B12 and D also way low.

LH: 2.9 mIU/ML
Prolactin: 7.4 ng/ml
Estradiol: 21.0 pg/ml
Testosterone: 245 ng/dl
SHBG: 17 nmol/L
Free Test: 6.2 ng/dl

Woman NP wants to do pellets (plus LDN for my alleged Hashimotos plus vitamins). I said I'd talked to friends and read up and preferred injections. She said something about pellets test being "bioidentical" and not needing to use AIs or something to reduce conversion to esteogen, while injections aren't that plus you do injections 1x weekly and get peaks and troughs. What about 2x weekly? Well, we have good results with pellets but might consider injections if that's what you really want. Pellets are really the way to go.

Leaves me with a case manager or something to tell me the bill, and they wanted $4800 annually just for the pellets alone. Gtfo bitch.  Tell me the price for injections. Case manager goes out for a few minutes, says NP told me to get a scrip from my PCP if I want injections.

Yeah, turns out the pellets are $750 apiece. Fuck off. I got an appointment with a buddies clinic next week. He does 2x weekly self injections. I'll ask about the thyroid thing too. Appt with PCP is in 2 weeks and will discuss with him too.

I left thinking about the Big Lebowski funeral home scene. Just because I'm bereaved doesn't make me a sap lol.

Looking forward to next appt with new guy. This sucks but I appreciate this thread. Read the last 25 pages or so and helped get me prepped to not be bamboozled by that clinic.

Not only did she lie to you about the bio identical and AI , but she wanted the huge markup for pellets which cost her $5.

Did they test your thyroid?  If they didn't and told you that I would report them.

I would have asked the NP to come back in and talk to me.

I wouldn't have paid her.

Yes, they did test thyroid and some of the numbers were slightly out of range.  It was just weird that I'd never heard of Hashimotos until the last 3ish years and now suddenly I've talked to 5 women who all have it lately. Dunno if it's a fibromyalgia catch-all or what.

And yeah the pellets cost is a scam. I walked out without even the vitamin shot or supplements or whatever, so didn't pay them anything aside from the blood work itself (paid upon drawing).  I didn't even want to talk to the NP again because I'd already decided I wasn't going to let them do anything to me no matter what else she said.

I'm taking the bloodwork to a men's clinic tomorrow. Everything my buddy tells me about this place lines up with what I've read in this thread as good practices.

Well I enrolled for treatment at a local clinic. Will be one injection per week.

For those who are on TRT, how long was it until you started feeling better?

I’d split it into two doses.  Less peak and valleys.

About a couple weeks.  I felt great. Still do

Had my consult today. Doc said “damn no wonder you feel like shit”.

It's probably been answered here already, but I'm going to ask rather than attempt to dig through 500 pages.

Is there a go to place for online prescriptions for trt?

I have been on try for 15+ years, know where my levels should be, know how to keep my blood work in check myself, and don't need any real help. My problem is I work too damn much and can never get time off for a dr visit every 2 months, and end up letting it go too long without injections. Which obviously is terrible for keeping my levels consistent.

thanks for any advice

I did TRT nation initially. Then switched to insurance model.

TRT nation was pretty easy.  Sign up. Pay.  They send you an email. You go to labcorp (thousands of them across the country) they draw blood (ten minutes)

Then you setup an appointment over the phone.  It was about 3 weeks out.
Guy calls you.  Says he wants to put you on test.
Shit gets delivered in the mail.

Sounds pretty straightforward, I'll check into it, thank you.

My job makes it a nightmare to take time off, and I can rarely hold an appointment as I am always offshore.

Without searching 400 pages, anyone ever just try to use TRT temporarily. Maybe to boost levels and get motivation to stay in the gym. Low energy levels, lack of strength, and fear of staying on it long term.  Not afraid of what it would do, but just don't want the commitment.   Maybe use 1-3 months and stop? Maybe when the weight drops off I'll be more motivated to stay going to the gym each day.

The short answer is that no you definitely shouldn't do that.

When you add exogenous testosterone, your natural production will shut down. You can't supplement your natural production with exogenous T, you must fully replace your natural production + whatever additional T you wanted to add. If you stop the exogenous T, your natural production will generally rebound, eventually, but you will have a very rough few months and loose any benefit (and likely more) than you'd have gained.

Do Dr will do phone calls and another does a check every 6 months.

Try Jordan Grant .  He may not be taking new patients but it's worth a shot.  He's a former urologist who now does TRT full-time.  He's also a major contributor to the TRT & HRT Youtube channel.  Since you are in Texas, you might be able to do everything virtually and never have to make the trip to see him (He was in Paris, last time I saw him).

He'll prescribe Testosterone Cypionate for injections, but he's a big proponent of compounded cream.  Also, he doesn't take insurance and instead charges a monthly fee for concierge service.  It's worth it to me, considering the alternatives.  The few times I've messaged him, he's responded ridiculously fast.

I just stumbled on this Q&A with Jordan Grant that was posted a few days ago.  It give a good sense of his philosophy for TRT.

Agreed.  His best option if he wanted try something like that would be to supplement with HCG.  That too would carry the risks of a decline in natural production over time.  The body has a feedback loop and wants the values to fall where it expects or it starts shutting natural production down.  One month on and then several months off might work for a mild "boost" but it is a poor choice.

The only way to get a boost that doesn't cause a decline is with clomid.  That takes awhile to work and has side effects such as elevated estrogen but it boosts natural production.  Clomid breaks that feedback loop and the body ramps all hormone production.  When you go off the clomid the body slowly reduces production back to the level it start from.  I tolerated clomid ok but the side effects did get worse the longer I was on it.  Others had major problems with it.

I've been seeing him a lot too.  He hates AI's and loves creams.  I've watched maybe 7 or 8 vids with him, super interesting stuff.  Wish I was in TX.

I'm very fortunate to only be one state away.  It's worth driving to Texas once a year to maintain the face-to-face requirement for treatment. He does prefer compounded cream but has no problem with prescribing Test Cyp, if that's what a patient prefers.

Gil T, who was a more prolific poster/commenter on that channel but has stopped doing that, has a very similar philosophy and offers treatment in all 50 states.  When I used him quite a few years ago, he had over 9000 patients.  His service is much more expensive but includes labs and basic meds.  The problem is you get treated by one of his PA's or Nurse Practitioners.  Doctor Grant will spend so much time with you in your in-person visit that you start to wonder if he even has any more patients that day.
