If you look at the MN/WI parts of that, the blue counties were some cities, but more FSA unions, indian reservations and people who live so far out in the sticks they need a democrat in office to get them their free fiber optics wi-fi.

The map in the op is incorrect. Obama carried 4 counties in KY none of which were in conjunstion with another in location. I have seen a current map and it is equally illustrative of how a small number of counties nationwide put Obama back in office though.
