Is there a roof or awning above the door? Could be water getting in from the top of the door frame.

I do this to exterior doors and windows once they are screwed in and checked for square.

Expanding foam won't do anything for water infiltration.

Thats basically used for insulation. Nowadays, its actually best to use caulk/backer rod to seal up the windows if money wasn't an option.

No pix from outside, but I'm guessing that there is no silicone or vycor behind your 908 casing, and likely no dripcap on top, since you say its just temped in there. Also, it looks like its only a couple inches higher than outside.
If its temped, there is likely no membrane, nor any silicone underneath the threshold.
Guessing here.

We never wrapped on a slab. We just put PL everywhere to seal and “bed” the threshold. MAYBE put down sill seal and usually had some green- treat to keep it off the slab.
At least the door is flush with rock!

This happened to us. Water coming in under bottom plate. Had to take door out, caulk the shit out of it and put door back in.

You need to install the corner pads on the bottom of the frame leg where it meets the sill. They should have been sent stapled to the side of the jamb. I would also recommend pulling the door and re-installing it with a sill pan.  Here’s a link to Endura’s corner pads since your door has an Endura sill.  FYI- 95% of doors that leak in the bottom corners don’t have the pads installed which is a requirement for most door manufacturer’s warranty.

I’ve seen that happen when the lower threshold was installed backwards.  Like the part that is supposed to be directly under the door was not. I can’t really tell in your pics if that’s the case or not.
