Normally it means the miles you've clocked on the interstate, within that state.  Normally runs south to north, on vertical interstates,  ie exit 1 on a north bound highway is the southern most exit.  In Alabama, exit 365 would be your northern most exit, just prior to crossing into TN.  On east -west interstates, exit one is usually the eastern most exit.  I believe they have standardized this on the US interstate system, but it wasn't more than 10 years ago that on some Pennsylvania roads, the exit numbers were just a numerical progression, with no relationship to the relatvie distance traveled.  I haven't driven through another state that numbered exits like PA used to though.
