I installed an Apex FSS a while back in my M&P9 it has the incorrect sear block with the 1/16th hole, but I installed it anyway.  Sometimes the striker does not reset, I am assuming this is due to the incorrect spring and plunger size?   The instruction say it will cause sear flutter,  I do not know if I should send it to Apex or try and get the correct parts myself.  I have looked a few times but I am not sure what block I need, I have a No thumb safety, no lock, model with RAM installed also.

I have the same setup with exception of the RAM.
Runs like a raped ape and have well over 8K through it like that.
I keep hearing tale of the elusive sear flutter on this configuration, even challenged a few folks asking for first hand accounts and got no response.

Have you inspected the striker and spring or tried it without the RAM?

When I first installed mine I had to adjust the trigger bar a bit until it worked properly. I couldn't explain it very well here, but just adjust one extreme to the other until you find which direction you need to go with the adjustment, then fine tune from there. It will mean taking things apart numerous times, but you can get it right with time.

Either that or send it to Apex, which isn't a terrible option.

You adjusted the trigger bar  because it was not reseting the striker?  I guess that is why I am getting a dead trigger.  Only happens ever 50 rounds or so in rapid fire.

Yep, I was getting a dead trigger, but in my case it was dead period. It took some fiddling to figure it out.

I'm more in line with the other suggestion of the trigger bar.
If it was a weak spring or broken striker it wouldn't be intermittent.  

I had similar but opposite problem when I first installed, ran like a scalded dog, but dead trigger on slow fire.
I had bent the loop/tweaked the bar previously, so i replaced it with a bone stock, unaltered bar and it's been flawless since.
Had actually forgotten about that until the other poster mentioned it.... My money is on the trigger bar now.

ETA if you had chaned the geometry it may be affecting the RAM also, so again...trigger bar.
I'd take the RAM out and try it, and/ or put a new bar in.
