Someone's terrible reload? ETA: Looks like an older model with "stainless" on the slide as well as that barrel type. I'm guessing a high round count with the amount of plastic worn away by the frame rails.

It was a range rental gun that literally had tens of thousands of rounds through it.  Factory ammo.

Yea,. I doubt the tens of thousands of rounds played a part in this.

The M&P is old enough now, that if this is the case, then it would be a daily occurrence.
