I'd like a 1911 frame that is forged from carbon steel, a full length dustcover (ie flush with front of slide) for a 6" slide, and a 1913 rail on the dustcover.

Where could I find this?

I thought SA on its website used to show options for longer slides, longer dustcovers, heavier slides, etc, but it is no longer there. Maybe my memmory is playing with me.

Wow, I didn't think people would make damascus steel guns.

What's the story with the external extractor option, and would a titanium slide hold up?

The extractor is one of the weak links in the 1911 design.  It has to be tuned (read bent) to ensure problem free extraction.  Once its set, it normally doesnt need it again unless you start to experience problems or the part has seen a lot of use and is too worn to hold tension.  The spring loaded external extractor cures this.  Not that Ive had a problem with it with my guns.  Mine get checked out periodically, and Ive only had to replace the extractor on one Springfield.  It was used for bowling pin shooting, and had an extremely high round count, with a lot of +P's through it.  I tried beating that gun to death shooting it and it still keeps ticking along at 100%.

I have no experience with the titanium parts other than having had a titanium hammer and sear in a comp gun I used to have.  It broke and got replaced with tool steel.  No idea about the slides.  Check out the Caspian forum at www.m1911.org.  That probably has some info on them.
