I need to send in the yearly MG registration form and have a few questions for those of you who have done this before.  
-How much of this form needs to be filled out?  
-Do I need to include the seller’s information at the bottom?  
-Where canI pick up duplicate forms?

I don't want to F this up, so all input is appreciated.

My dealer did mine when he completed the transfer, to renew I just check off that nothing changed. I don't think the seller's info is needed.
I'm sure their website has more info/forms.

I just checked the website.  Apparently they are supposed to mail me a new form each year.  So far I havn't received it, probably because I moved.  I did update my address with the DPS; so who knows.  
The MG registration form I have now doesn't have the "nothings changed" option.  It asks for all the information, including the original authorization number, sellers date of birth, sellers dealer/permit number, etc.  If the new form doesn't show up in a few weeks, I'll just call the DPS.

How does this work if someone were to move to CT, and already owned machine guns?  I very much doubt in some cases that one could track down the seller to obtain their DOB.  What about MGs obtained from a Corporate FFL/SOT?  A Corp. does not have a DOB.  

The company I work for is based in CT, and I'm considering switching positions to one at the home office.  But the more I read here, I can't help to note what a F-ed up state you all live in.

Follow-up question:  From the CT Licensing Info website, it notes that "Any Connecticut resident who owns a “Machine Gun” is required to complete form DPS-418-C, registering that “Machine Gun”..."

Is a non-resident allowed to bring a machine gun into the state?  Anything special that needs to be done beforehand, other than to configure the gun so it is not "select-fire"?

You need an approved ATF Form 5320.20.  On the 5320.20 that you submit to the BATFE, state that the machine gun has been rendered full auto only in order to comply with Connecticut law.  BTDT

Call the CT DPS Special Firearms and Licensing Unit and ask to speak to Deidre (860-685-8400).  I moved into Connecticut with a machine gun.  When I registered it with CT DPS, I put "not available" on the registration form for the seller's DOB.  For a corporate DOB, I would write "not applicable" but give the SLFU a call.

When I lived in CT, I received my annual form in June.  After the first time, I just checked "nothing has changed."

I doubt that DPS would make you track down all that information.    They may even have different registration cards for residents moving MGs into the state.  The card I have asking for that information turned out to be for first-time purchase.  I was confused because last year the dealer gave me a bunch of them and told me to mail one every year.

What REALLY happens is once registered, DPS sends a piece of paper (I got mine yesterday)listing all your MGs with their basic information, and if nothing has changed you're supposed to write "no change", sign, date, and mail it back.    

In reguards to temporarily bringing MGs into state, I'm unsure if they need to be registered with DPS; someone else probably knows more about this.  MGs brought into CT need to be made to fire FULL AUTO only.

On the BATFE Form 5320.20 you state whether or not the movement of the machine gun is a permanent one or for a specified period of time.  If the machine gun is going to be in Connecticut temporarily and you are not a Connecticut resident then AFAIK you would not need to register it with CT DPS.  It cannot be select fire while it is in Connecticut.  I moved into CT as a resident so I registered my machine gun when I arrived.  Call CT DPS SLFU and speak with Deidre to get the straight scoop.  I've spoken with her a number of times and she is very helpful.
