Hey guys,i am having my wife apply for a FOID card and she has never had one before. Under "List Any Previous Names" should she print in her maiden name or leave it blank? We have been married 25 years if it makes any difference. Also should she fill in the optional daytime phone number with her cell phone number?
I figure someone on here has experience with their wife's FOID application. Thank you for the help. |
