Quite sometime ago there was discussion on reproducing weapons cards. IIRC, it was the type the Army used.

Here are some examples of what the USAF used in the mid-1980's

Looks the same as ours in the Army in the  70s.  Until recently, I had my weapons card from the 54th MP Co. at Fort Ord!  Thanks for posting.  At one point I had one for an M16A1, 1911A1, and 2" S&W Model 10 .38,

Thanks for posting.

I was USAF Security Police for 21 years.

I always kept my weapons cards in my M-16 ammo pouch
so I always had them when checking out my rifle.

I was an SP for 11 years then retrained into disaster preparedness. Kept mine te same way when I was not working the armory.

Edit:  The links are for a similar DD form, not the USAF form above.  I just played with the fillable version and read the thread for the first time.  Pretty impressive work flatdarkmars and others that helped!

I recently built a M16A1 for a long time friend who told me he wanted me to build a rifle like he carried in Vietnam. When I gave him the rifle it also had an arms room card filled in with his old unit info, service #, etc. He was speechless for a long time.
