Joseph Chiarello does a lot of gun industry insurance including instructors & ILEETA has a program for members if you can demonstrate at least one of the company owners is a law enforcement trainer.

JC only covers shooting- no FOF/H2H programs.  The ILEETA program will cover about anything.

I second YammyMonkey.  We use Joseph Chiarello for our insurance.  It's 350 per instructor per year for a million in coverage.  Great company.  Very helpfull for startups.  Talk to Toni Rouse, tell her we say "Hi"  from Tactical Training Associates  www.WestCoastTacticalTraining.com

Not to pimp my own site (forum for instructors), but this came up over there http://instructorcommunity.com/index.php?topic=42.0. You have to register and be set as an instructor to see it, so I'll say that http://www.guninsurance.com/index.html was recommended by a reputable instructor.
