The system test failed.

When they accidentally sent the wrong message there was no way to issue an order to cancel in the system.

They didn't have a way to send another message that it was a false alarm. They had to re-issue it manually which took 38 minutes.

Most of the sirens did not go off during the false alarm.

After they issued a false alarm 38 minutes later then the some of the sirens started to go off throughout the city.

Clusterfuck.
